Registration Forms
Once you have reserved a spot online, the registration forms below must be submitted. A separate medical information form must be done for each child, on a separate sheet. The other forms have space for multiple children.
Print, fill out completely, sign, e-mail images (kkokjer@ieee.org), or bring to first day.
